2017Registration materials:

 

 

 

  • Concussion Materials
    With Oregon's new law SB 721 (a.k.a. Jenna's Law), not only are Coaches and Officials required to be aware of concussion symptoms and procedures, but parents and players too. The OHSLA requires that the following documents from the Center of Disease Control (CDC) be read by both parents and players.  The form in the last document must be signed by the player and at least one parent and returned with registration materials.

  • Fact Sheet for Athletes

  • Fact Sheet for Parents

  • 2017 OHSLA Concussion Acknowledgement << this form must be signed by both player & parent

 

  • Concussion Baseline Testing
    Players are required to complete concussion baseline testing every two years. Baseline tests from other sports meet this requirement. The Portland Clinic provides baseline testing, paid for by the club. See the following flyer for details.
